- [Narrator] Here's how to create a new form from scratch. From the homepage of the forms app, select new form. The form auto saves with the default name, such as untitled form that you see listed here, as well as in the field here at the top. To change this name, select this field area, and then replace the name. An optional image from a Bing image search, from OneDrive, or a local computer upload can also be included by selecting this picture icon. Next, let's give the form a description. We'll select the form description field and enter the description. And that's how you start the process of creating a new form.
