- [Instructor] Need to gather feedback with a number rating? Start with the form open, and then select Add new. From the question types listed, select Rating. Enter the question text in the question field. If media needs to be included for this rating, select, insert, paste, or drag media, and then you can select an image or a video. This is an optional setting. Next, select the rating levels. It's currently set to 5, but you can set a choice of 2 to 10. We'll keep it at 5. You can also choose the symbol. It's set to star right now, but there are several that you can choose from. We'll leave that set to star as well. If this question is required, select the Required switch. And that's how you can add a rating question.
